Help? All four O2 sensors showing low voltage...

I'm about to do a search, but thought I'd post to see if anyone has dealt with this and what the cause/fix might be.
Thanks in advance for any help...
After searching, it looks like there could be a number of causes but I couldn't nail anything down for sure. I'm hoping someone out there has dealt with all 4 throwing a low voltage code.
I'm going to check visible wiring and go from there. Any help is still much appreciated. Thanks guys.

Hmmm...I'm not real familiar with HP Tuners, but it was throwing the PCM into 'safe mode' and the timing was like 19 degrees or some such thing. Not sure what else to offer. It was running way lean, but I forget what the numbers were.
