It means your cam timing events occur 4 degrees sooner in relation to the crank shaft.Advancing the cam will improve low rev performance,while retarding will improve performance at higher revs at least in theory,in actuality other factors such as head and exhaust flow come into play.Since most of our trucks are driven on the street at lower revs advancing the cam is a better compromise between power and drivability.Advance can be ground into the cam itself or set at the time of installation by means of an adjustable timing set or offset dowels.A degree wheel and dial indicator are needed to check cam timing at the time of installation.

Adjustable chain.
Advancing the cam makes the valve timing events happen sooner, decreases top end hp but increases bottom/mid torque. Did i say that right.
My cam lost 9hp but I gained more in the low to mid power band where i use it most.
