The shaft is the crank snout. It is supposed to be like that. Your bolt wasnt tight enough and, like said, the pulley came loose, wobbled around and now you need a new pulley.. and bolt.

I may be out of the norm, but I dont recommend Lock tite for the bolt. If you use a NEW GM bolt (like youre supposed to), the red stuff under the head is a type of "Lock Tite". Also, if the bolt is TQed correctly, you shouldnt have any problems. Ive had many a crank bolt on and off and havent had issues. I actually switch the the ARP bolt. Worth the cash.

Check the crank snout to make sure it didn't wear away any material. Honestly I'd be tempt to just reuse both lol. It's not a true balancer, its a dampener. As long as it presses on tightly and you can torque the bolt on pretty tight, you should be good to go.

ive seen it happen with asp pulleys before. you have to tighten the bolt as tight as you physically can, then get a longer cheater bar and get someone to help you tighten it some more lol.
your asp pulley is junk now, and your crank should be fine.
