Thinking about it now though it isn't the PE that is causing this to go into 8 AFR. It is all based on throttle position.
I'll probably tinker with it a bit tonight if I have time.
Edit: Is my commanded AFR supposed to be 14.26 or 14.7? I noticed that it was at 14.26 in that tune.

In the log you hit 77.2 % TPS so yes you were in PE
The tune I'm looking at is "turkey dinner" and the stoich is set at 14.12 which is rich. Ideally you want 14.7 but with shitty ethanol in today's fuel it's probably around 14.63 with 10% corn. You can bandaid the PE by lowering the numbers. Tuners (which I'm not) call that "raping the PE table".
PE is a multiplier of stoich so when the fuel tables are correct...
14.7 (stoich) divided by the PE table 1.210 (a number in the turkey dinner PE table) you should be seeing 12.14 fuel/air in PE which is acceptable.
With your fueling tables off it throws the PE multiplier out of whack. It multiplying incorrect fueling tables that's why you're seeing those super rich numbers. That's why I said to work on the fueling then the PE may fall in place without a heavy doctoring of the tables.
Is there a previous tune where the truck ran good and the wideband was not so far off?

Here is kind of what I am doing tonight, I will attach a few tunes and logs. Following goat rope garage's videos I believe I am on the right track. The truck is starting to idle and drive better with every adjustment.

Okay here is my final tune of the day, did not drive on it but it is based on this last log. You can see my text doc of changes.
My two big things that will change this tune pretty heavily are the stoich and PE. I have PE set way off to 90% throttle so it does not interfere with the MAF tuning. Stoich I have it set at 14.12 based on what I read on the HPTuners and LS1Tech boards but I am willing to bet 14.6 would be more accurate since I do will not always have E10, most likely E5 lol
One other thing I should mention is that I also plan on doing the 02 OS at some point.

The idle fueling is close but you're getting quite a bit of KR at part throttle that's pulling time. It's not showing lean in the largest KR area on the scanner chart.
You need to figure out if it's false knock. Pull around 4 degrees in the area highlighted and see if it helps the part throttle knock. If it has no change it's probably false knock. You'll have to make the decision to desensitize the knock sensors (I had to desensitize but I tested with octane booster to be sure). You did mention you may have a tank of bad fuel. That could be the source... you need to do some investigating.
Don't forget to add those degrees back to the timing tables when you figure out the issue.
Then keep logging and smoothing out the fueling tables, you're on the right track.

I've dealt with the false knock before and my solution was turning off the codes for the knock sensors but I don't think I desensitized them. I will pull timing there and then do a log or two and see but I most likely will end up desensitizing. 99-00 OS is garbage tier I swear. I did check for knock and I did not see any when I was looking, do you have a screenshot of where you saw it?
I still haven't fixed my header touching my frame so it could be that as well. OR it is my bumpy *** roads getting out of my neighborhood, seriously that makes me not want to drive this truck at all since I don't have a notch. They tore up the main road and did a half *** job repaving it.

If it's false knock you need to desensitize the sensors. They will still work unless you take to much out. IIRC you raise the numbers in the table. If you hover the cursor over the table button it will popup the info.

Damn I even looked around for that too on some of my logs must be blind. That is quite irritating
I may just need to get on the 01-02 OS. Probably buy a PCM and keep my 99 as an extra and for my last emissions test next year.
I can see that my AFR right there was 12.09 so I don't think it is a lack of fuel knock.

Pull the time out of those areas, make sure you're not burning bad fuel IIRC you thought you may have gotten a bad tank, boost with octane booster... if the KR is still present put the time back in, desensitize the knock sensors. Then get back to logging the fuel tables. If you swap PCM's you'll be dealing with the same issues along with a lighter wallet.
I'm running in open loop (no o2 sensors trimming the fuel) with the custom one bar "Speed Density Enhanced RRT" operating system. It works great for my cam with a lot of overlap. In closed loop the o2's always pull me slightly lean because of the overlap contaminating the mixture... so I said "fawk the o2's".
