Front shock install help!

I'm working on installing Bilstein HD shocks on the front of the 2005 Silverado Z71 Ext cab and I have run into a possible problem. The mounting hardware that came with the new shocks is different. My old ones have a insert in the middle of the bushings that helps fill the hole of the top mount.
Am I supposed to use my original bushings and plats on the top of the shock?

You won't use the sleeve in your new bushings. If you look at the bushings, one side of each bushing will be smooth, the other side of the bushing will have a "step". The "step" will center the bushing in the frames hole.

By the looks of this not so great intall guide I think I went to tight. I have a decent amount of bulge on the bushings. Wonder if it will ruin the lock nut to back them off?
