Default Broken Tensioner Pulley

Some of you might have read my post "Procharger Pics" and looked at my broken tensioner pulley. Earlier tonight I finished installing my 60 lb injectors and went for a test drive after tuning. At full boost my tensioner pulley decided to come off again. According to my boost gauge I reached 10 psi.... This time it went through my radiator ... Does any one know what could be causing this?

Did it come off when you release the throttle or first mashed the throttle? Belt the correct size? Hole where the pulley mounts striped out?

When I first mashed the throttle, I noticed that it broke again...I pulled over and killed my truck as fast as possible...after looking at it it broke completly off, (the three bolts are still in the water pump)...the rest of it took out my e-fans and my radiator...the belt is the one procharger sent me and it was nice and tight...I am confused and wondering what is causing the tensioner pulley to break on me twice? Kinda frustrating lol....

Pics of the broken parts/brackets? Also, pics of the setup as assembled(before breaking).

The only pics I have before it breaking are posted under my post "procharger pics" a few days ago...I'll try to post up pics of the broken parts tomorrow.. it just completly broke the tensioner pulley off, but the three mounting bolts are still secured in the water pump...

Ive had problems with breaking tensioners also before. What I did was I purchased some black grade 3 or 5 bolts and flat washers and replaced the bolts and a new tensioner from oriellys and have not had problems for about a year now.
