Sure, you can use stock heads. For strokers though, most will get ported heads to take advantage of the extra cubes. Now, a 3.75" stroke might only be achieved with on off-set ground stock crankshaft. 4" pistons? Not in a 5.3 block, they can't be bored that far. You're better off buying a 6.0 block. Everything from your 5.3 will bolt right up and fit right in to your engine bay. Bore a 6.0 block .030"-.040" and you get 370ci with your stock crank. Throw a 4" stroke crank and you get a 408-409ci engine.

No, you can not get a 5.3 to 383 ci. You will need to get a 6.0 Truck block throw a 3.75" crank in it and bore it out .030" and you come out with 383 beatiful cubic inches. You will need piston's to fit, 6.125" aftermarket rods, rod bolts, gaskets ect...all the other goodies. You can keep the stock heads, but if I was building a motor, definitely would get a good set of heads.

With all this I would look for 480+rwhp all motor with the right combo.

Granted you will need a good ported intake/tb/exhaust setup to do this.
But, this motor with the correct tune would be bullet proof and could take you upwards of 1000rwhp with the correct FI setup.
Cost would be upwards of 5k.

But, you will need to get a 6.0 lq4 truck block to do this.

you can get a 383 out of a 5.3L block.....just bore it out to accept 5.7 pistons from LS1 (i think bore is like 3.8xxx) so basicaly you have an iron block LS1, then put you a 4" crank on that dude and you got yourself a 383 stroker motor.....port and polish your 5.3 heads and mill them about .010 and i believe that would give you a compression of about 11.0 to 1, now if you dont want run N/A and you want to do a turbo or sc then get you some ported and polished 6.0 317 heads to lower yoru compression in the upper 8 lower 9 range so you can run boost safely.......wouldnt say it would be that cost effective though bc you could prolly build a 408 close to the same price and make more power out of it.......

(edit) I was typing during the previous response...

Sure you can. You can bore a 5.3 block to 3.9" (stock LS1 size) and put a 4" stroke crank in. That will get you to about 382 cubes. I've stroked otherwise stock LS1/6's to 382 with no problem... even with Forced Induction. Last I checked there is no 3.75" stroke crank for GENIII engines. The stock 5.3L motor comes with a 3.622" stroke crank... same as LS1. I believe typically available stroke cranks for the GENIII/IV platform are stock stroke (3.622), 3.9", 4", 4.1", 4.125", and 4.250". Certainly a custom stroke can be obtained but... why.
How much it will cost will depend on how much you are willing to spend. There is a big difference in pricing from, say, Eagle products to say Callies and Oliver. Good engine builders are all over the map on pricing. Figure at least $4k from a reputable machinest to build a stock 5.3 shortblock and stuff it with stroker parts... and that does not include any sort of shipping costs assuming you go with someone out of your area. Yes, stock 5.3 heads are fine... but again it depends on how much money you want to spend. If you are going to the effort of stroking a motor, especially if you will keep it normally aspirated, I would have someone like Richard at West Coast Cylinder Heads CNC those heads and install good valves and springs. Expected power levels will be dependant, for the most part, on what your expectations are on reliability and longevity of the engine. If you want 400rwhp then I would expect the motor to live the life of the truck. If you want 500rwhp... then expect periodic indepth maintenance. Also, the higher your power expectations are the more money you will need to spend on the rest of the drivetrain to be able to handle the extra power with any sort of length of time.

seems to me that if I was going to do all that I'd just get a 6.0.
