A lot of people go with Pacesetters, but the Ebay OBX/pacesetter knockoffs are proven performers as well for 260 + shipping

When I first bought my LPP's, I had the stock Corsa on my truck. They bolted right up, and the Y pipe fit perfectly. They are also stainless to boot.

Depends on how much you want to spend. American Racing headers are expensive, but work great. No leaks, and the install was easy.

I went with the dynatech's and a catted y-pipe and all stainless. Pricey system, but top quality IMO. Haven't had any leaks or other issues.

What are your future plans for the truck? I'd recommend getting headers that you can "grow into", and of quality that will last for years and years of service. It's hard to beat a well-built stainless header, but depending on your needs, you may be over-spending.
