When the engine is cold, the fuel injector's spray has a much greater tendency to cling to the walls of the intake runner, to the valves, to anything metal on it's way into the combustion chamber. When you give it gas when cold, a large portion of the fuel sticks to the intake runner walls than makes it in, causing the very lean condition. Once the engine heats up after a couple of minutes, this occurence is not longer a problem.

Picked up this anecdote in the EFI 101 class.
