Default Accelerator pedal drops out occasionally, P2133 P2139 P2140

2005 LLY Duramax - I've had an occasional problem that's left me on the side of the road twice where the accelerator pedal stops responding. I have my V2 hooked up, and when this happens the throttle position reads 0% regardless of pedal position.

I scanned the ECM and have the following codes:

P2133 - Accelerator Pedal Position (APP) Sensor 3 Circuit High Voltage
P2139 - Throttle/Pedal Pos Sensor/Switch D / F Voltage Correlation
P2140 - Throttle/Pedal Pos Sensor/Switch E / F Voltage Correlation

Has anyone experienced this before? I can try to troubleshoot by checking voltages and such but being an intermittent problem I am not sure that I will find anything unless I happen to get lucky and catch it while it's happening.

Sounds like a bad APP sensor or shorted wire with 1 signal being out of correlation with the other 2. I guess you could log the 3 APP sensor signal voltages to see what they are doing. Can you view the freeze frame data from when the DTCs were set?

DTC Freeze Frame Data
The PCM may save a Freeze Frame (snapshot) of some parameters' values when a Diagnostic Trouble Code is set.
Up to six Freeze Frames are stored in the PCM. If more than six Freeze Frames are saved then the oldest frame is discarded first.

Screenshot of the APP sensor voltages. This was done with engine off. Sensor 3 voltage stays at 5v regardless of throttle position, but in the freeze frame data above it's not 5v. Does this tell me anything?

I just took the connector apart at the pedal and probed the sensor 3 signal wire. The position sensor is working correctly, the voltage shown on my multimeter is different than what I was seeing on the scan tool. I disconnected and reconnected the under-hood connector and got signal to the ECM again for sensor 3, so I think it's just an intermittent connection issue at that connector. It's working again for now, will see if it happens again.

If the ECM is seeing 5V the signal circuit is shorted to a 5V circuit....should never go all the way to 5V. Maybe you got a Toyota sensor...LOL Tin whiskers

I have switched pedal harnesses and pedal position sensors. Now I'm getting P2138 and P2140 instead of the original codes. I just replaced the pedal position sensor a few days ago, so I am going to drive it like this and see if it still cuts out. I logged voltage on all three circuits and they are all getting signal, so I'm not sure why the codes are there. SO far it seems to be driving ok though. If it still has issues I am going to have to trace the wiring from the ECM to the pedal harness connector under the hood and see if there's a short or damaged wire somewhere.
