The idiots at Cutting Edge Customs used a carrier shim from a 98 and down 10 bolt - they have smaller carrier bearings and shims. Well that shim ended up inside the axle tube, allowing the carrier to shift side to side which caused the ring gear to strip. It was more than obvious what caused the gear set to fail.
