Default My truck hates me....

Well i took the truck to a fairly local shop to have my driveline angels checked out to rid myself of the oh so annoying vibration i have from 75-95mph.
While test driving the truck at about 6500rpms in second gear suddenly it makes a very broken bad sound then i hear something bang around under the truck as if something broke off and flew away.
Truck is now running badly and sounds horrid.
Get it up on a lift and the EGR bung weld in on my dynatech headers broke off.Yep gone....
Well i was told i need a 6degree shim to rid the vibration.Cool no big deal figure i'd pick it up Thursday on my way to work and change it out. Easy job takes less than 30mins and thats setting up and picking up.
Wednesday i took the header off the truck and brought it to a guy local to me and had the hole welded up and looking nice again.
Put the header back on and the truck was good to roll.

Later that night my GF had to pick up her kiddies from daycare. I told her take the truck blah blah. I get a call 20mins later.
Truck broke only reverse works.
So i drive to her and look around and what not and sure enough the truck will go in reverse and move without any trouble but all forward gears are useless.
About 2 weeks ago i was leaving for work and very very mild peddle it shifted into second and suddenly free revved up to about 5000rpms. I let off coasted and gave it a little gas and all was ok.
I guess that was my warning something was wrong but just figured it was something random and moved on. I mean it was a couple weeks since then and it ran perfect.
Anyway now my truck is down with a broken tranny again this makes the fourth time now =]
Any ideas on what broke in it this time?
Didn't really hear any funny noise come from it....

That and the stock ride height has been known to cause nosebleeds.
