EATON 19559-010 is what's on my invoice. My truck is a 2000 Silverado. I am not sure if the 2004 is the same, but I don't see why it would be different. eerrr I take that back, I know nothing about 2004+ trucks.

I bought mine June 2004 from Drivetrain Specialist out of Las Vegas for 340 bucks. You need alot of crap to go along with that, bearing install kit, ring & pinion I am guessing, you are gonna want to make sure your installer puts in an "additive" for the posi. I haven't shopped them but I'd imagine they are under 400 hundred bones.
