I think this would be hurting you the most.

I'd slap some milled 243's or some aftermarket heads on there with some 8.1L injectors. Those heads with a cam that big may be choking out the motor; not letting it breath as it needs to. That also may be why your IDC is only at 95%. I would think that a cam that big would be making those injectors cry unless the heads are maxed out, and it's not getting enough air to max out the injectors.

Just my .02

Injectors are hurting you, even at 95% they probably aren't efficient for very long.
The heads are hurting you but not to the point that you're only making 311hp. I'd check the tuning aspect. Also, what was the IAT and outside air temp/DA?!

Plenty of guys are making 320+ with smaller cams and the same setup as you i.e. LT's, TBSS intake, efans etc.

I ran a 224/230 .611/.607 111lsa on my old 5.3 truck. It ran great being cam only but the flow of the stock 862's didn't help the big lift of the cam. After I put the P&P'd, valve job, milled 862's on the truck was night/day different and the cam pulled ridiculous on the top end.
