I mean a leak on the exhaust before the turbo. Pull the rockers off and pressurize the intake with some shop air to check for leaks. Keep it in open loop and put the wideband in one of the narrowband holes to see what idle is doing.

I'm gonna throw this out there and you'll probably not believe me, but it's quite possible that you have a misfire/incomplete burn happening on one or more cylinders. An incomplete burn will A) Not be detected by Misfire and B) Read Lean on the affected bank and at the tailpipe.
I wouldn't recommend running under a lot of boost, but try swapping in some TR5's and seeing what happens. Have you measured spark output from your coils? Also, have you clipped a labscope to your plug wires and graphed the spark output? You could have a phantom dead coil that's dropping spark after a short burst. This can cause crappy running, hard starting, but not actually show up as lean.

Smokeshow - i wish you lived next door. What do you mean by "get a video of startup after writing a new tune to it" are you saying when i have to use brake clean to start my engine?

Yes, I want to hear what it sounds like when its struggling to start. Without the brake/carb cleaner, though.

One thing I would suggest doing is opening up the spark on your plugs. .025 is pretty small for a TR6, I am running .024 at 17 PSI with a 7 series plug. Right now, I would use the stock spark until you get it running properly then I would tighten it up for when you turn the boost up. I think Suburbanize is right that you are having a spark problem somewhere. There isnt your OS# at all on holdencrazy, even for a truck?

Are you sure you don't have the O2 sensor plugs switched? If you switch the banks, it can cause exactly what you describe - adding fuel on one bank and pulling from the other. Or, your intake isn't tight - as your pictures elude to...
Why isn't the tuner tuning it in open loop to start with? The O2 sensors shouldn't even be in the equation at this point...
