I have a feeling that the MAF is a big contributor to the issue.
I would try putting the stock one back on.

cleaning the maf will help too, they make a special cleaner now, but I think electronic contact cleaner in the spray can works too.

bank one and two lean usually mean the intake gaskets on these are leaking....get a new set....20 minute job......that would be what I go after first.

I think my GMS fried my cats, I havent hear of anyone else having them go bad at 89,9000.
