Here's the 5 lug front hubs. 99+ Silverado/Sierra 6 to 5 lug Converted Timken hub assembly. - $238.00 : 1320 Wheels
Any reputable company that makes rear axles will make them with whatever lug pattern you want. Rotors will have to be welded up and redrilled.
