It bolts to the trans. They wouldn't bolt to the body because that would transmit noise. I have only seen those on a 4L80 that uses the factory dual inlet muffler.

My RCSB does not have this. It is usually on a 2500hd bolted to the transmission.

well my 4l65e SSS has it. i believe any 6.0 factory truck has it. it just holds the slip fit and bolt up one. its a smart design everything moves with each other

Right, so since they made regular cab 6.0l trucks (2500 series) I'm sure there's a provision in the cab to bolt this hangar on, correct?

it bolts to the tranny. end of story. but they did make 6.0's with 5 speeds. in the 2500 hd. so i dont see a reason why it wont work.. or if not you can find one off a 5 speed one.

There are various mounts for the different transmisions that were available with the 6.0 & 8.1L engines with dual exhaust pipes. You just need to pick the correct style for the transmission and exhaust configuration you have. I believe I got the designations right, but one would be prudent to verify before ordering anything.

4L60 Mount

4L80 Mount

(RPO) MT1 C5F/C6P Mount

(RPO) MW3/C6P Mount

(RPO) ML6 Mount

(RPO) M74 Mount

it goes up over the crossmember and bolts to the side of the trans just like the image above of the 80e. i pulled mine off when i got my exhaust redone because they put other hangers on instead.
