Default QTP cutout

This thing started leaking a couple days after we installed it, about 2 months ago. I fired up the truck this morning and it was noticeably louder than normal, but not quite "open cutout" loud. I assumed a nut backed off the cutout, and it was just leaking badly. Heading down the highway, on my way to work, I heard something fall off the truck, and it got LOUD quick. I looked in the rearview mirror and couldn't see anything, so I just kept going. We put it up on the lift tonight and found this...

I've seen alot of guys complain about them leaking, but falling apart after 2 months? I guess I'll call em on monday and see if they'll replace it.

It appears that the butterfly loosened up and fell off...

It would never close fully, unless I held the switch closed. Otherwise, there was a 1/8" gap between the butterfly and the body of the cutout.
