Looking for a dual 3" into single 5" y adapter. 5 3/4" center on the dual side. I've searched google.

These guys dont have that size listed but I'm sure they could make one.

Parallel Merge Collectors - Columbia River Mandrel Bending

If they cant do it call up Stainlessworks or Burns stainless.

Glad to help... I've ordered from Stainless Works before and they're very helpful. They can do custom header flanges from a gasket or autocad drawing also.
