Spitting the tailshaft bushing out!

So my new transmission has a little over 1500 miles on it now and about a month ago my buddy was bragging about how fast his new 6.0 Powerchoke was with his custom tunes and I wanted to show him otherwise. We went out and I showed him my new taillights, but on the way back into town my truck started to smoke like crazy and the speedo went all wack. I finally got around to fixing it, the truck had been sitting and I was ready to drive it again. As soon as I lifted the truck in the air I saw that the tailshaft seal had fallen out (it has done this once before). I pulled the driveshaft out and the bushing was siezed on the driveshaft. The driveshaft is at the machine shop now getting fixed and the tailshaft houshing will need to be replaced because the hole for the bushing is egged out. Has this ever happened to anyone else? What would cause it to spit that bushing out? What would cause the tailshaft seal to fall out, it has happened twice and luckily I was within rolling distance of the house both times it happened so no harm to the tranny because of low fluid.

I honestly question the guy that installed the seal. Personally I have never had one blow out and I've installed thousands. The only reason it would just fall out is if the seal was installed with a hammer and its weakened to begin with. Or the housing seal area was cleaned up with a grinder of flapper wheel because it was damaged when a seal was removed in the past. If you need a new housing let me know. Vince
