Default Rear disk.

Ok so here is my question. I have a 99 astro and I wanna do a rear disk conversion but I don't wanna swap out for a six bolt pattern of the 03 and up from the astros. So as I understand some blazers from 98 and up came with rear disk setup. Would I be able to swap this setup onto my 99 astro? Maybe swap out the whole rear diff or something?

Any help would be apprecieated.
