Does this sound like trans failure?

I think I may have a dying 4L60. It just started while driving on the interstate with cruise on I feel the truck lurch occasionally. No appreciable rise in rpms or speed, just a small jerk and then the truck resumes normally. Also had to accelerate pretty hard to merge into traffic and the downshift felt a bit softer than normal.
Truck has 110,000 mi (has been modded since 15,000).

When you say "lurch", do you mean nose dipping (slightly) or as if you're going full throttle & squating the back end?
Reason I'm asking is b/c I've had 3-4 clutches wear, and cause no large rise in RPMs until I tried to go WOT. Gradual pedal apply got me through 4th & TCC lockup just like normal.
