Rick @ Synergy Motorsports has run 17lbs on a stock block 5.3l for a dyno run and runs 14 lbs all day long on the street (over 5k miles). Obviuosly, his tune is spot on and is running a ton of Meth.

His new kit hits 18 lbs.

Most other setups, STS trick etc. You can push 10 comfortably. But I would always get stronger springs and rods for added insurance.

Depends on how long you want the motor to last. I have mine set at 10psi with a max of 14 degrees of timing. I also run a 32oz can of Torco per fill up. Intention is to have the motor last the life of the truck. However I am going to build an LS6. My truck is my daily driver and I do not have a backup... so my expectation is it will continue to be at or near factory reliability until I am ready to swap the motor. IMO, more or less boost is inversly proportional to factory motor life. Some get away with higher boost levels but everybody drives their truck differently... not to mention most have their own tuner which also adds to the mix.
