Yes, new disks are required and no mods necesaary. Part of the benefit of doing the conversion is bigger rotors which means more mechanical advantage. But its more than just bigger rotors. Using a caliper with smaller pistons means that the pistons can be pushed out further to the edge of the disk. That gains more mechanical advantage as well. Everything is built around Baer Eradispeed rotors. Any of the site sponsors that sell Baer should be able to get them for you. Everything should bolt on and go without shimming. To my knowledge there has only been one or two people that felt the need to shim the rear. Occasionally some users have to grind a little excess material around the mounting surfaces from when the spindle is cast, but most don't.
