Well this has me confused? I thought of it as follows:

Bank 1 Sensor 1 = Driver's Front O2
Bank 1 Sensor 2 = Driver's Rear O2
Bank 2 Sensor 1 = Passenger's Front O2
Bank 2 Sensor 2 = Passenger's Rear O2

When I log them they read as front O2s B1S1 and B2S1.

if your o2 is old with some mileage, replace it anyway with the vette sensor. Won't hurt right? After that, if you are still experiencing it...it's the headers.

foff667...hahaha, dammitt you, trying to confuse everyone or what?!!?

Ok Ok. Ill go and get me a new O2 sensor for the drivers side. (the one in question is stock with 105K miles on it, and about 45k with headers. ) I guess its time huh? I checked the fuel regulator on the fuel rails and it seemed to be OK. Damn Im a dumbass.

Ok UPDATE!

I got the new O2 sensors in there and im still throwing fuel metering codes and now im throwing a MAP sensor code (maybe caused by the 224/226-110* i just threw in there. ) I did swap the heads a while back and it doesnt smoke or steam so i dont think im leaking water into the cylinder. It also has a hard start every now and then. What could this be? Fuel pump taking a ****? SOMEBODY HELP ME SAVE THIS TRUCK BEFORE I LIGHT IT ON FIRE!!!
