Default 6.0 cold start issue (only when cold)

I have notice a few weird things.
When it started getting cold out, the truck struggled to Idle and rev in park, and died when put in gear, then when the engine would warm up more, its just fine.
when you go WOT, it will rev to about 4500rpm and wont go any higher, it just kind of stutters.
It hasnt had a tuneup yet and has about 80,000 miles.
It has me stumped.
So
Stock tune nothing has ever been done to it.
Cleaned MAF,TB and all sounding parts/sensors.
Still the same problem when its Cold outside (30*-40*)
No tune up yet but plugs and wires would not effect idle only.

What year is the truck? Does it have a cable throttle body? If so, check the Idle Air Control (IAC) solenoid on the throttle body. If it is not opening, the truck would not be getting enough air and could cause your problem.
