Just went through the same issue with my 2011 5.3. Contact your nearest Caterpillar dealer. Get an oil sample kit, pull a sample and send it in. The results don't lie! Usually you have to sample oil at every oil change to get a good history on engine wear. But in this case you may be able to catch elevated particulate debris such as aluminum, chromium, cast, silicates and other materials used in the engine parts. If these particulates are elevated it may entice the dealer do a more thorough inspection.
Just a thought it only cost about 20 bucks.

So, dropped it off today at the dealer body shop for some minor insurance covered repairs.
Called the service manager [whom I know] and told him it was there.
Told him the noise is worse, which it is, despite the rise in ambient temps, let him know that I did a combustion chamber cleaning [the right way] to no improvement.
I live in a small city and this dealer is busy, like 140-150 appointments a day.
I fiqure if it is in the b/shop for the week, maybe they have more time to look at it ,
rather than make the routine assumption that it is piston slap.
As stated before, I was freaked about them finding tuning changes, but have satisfied myself, that once back to stock
there is no log of the changes.
I love my freaking truck, I hope these guys will give me a little time.

They had it in the body shop for a week. Service guy went in twice to have a listen
to the engine noise.
They told me again, it is an acceptable noise, it is not unusual and not bad enough them to do anything about.
He said it doesn't hurt anything, there is no downside.
BS, I won't buy this truck with that much noise,
I shouldn't have to listen to that much noise, it interfers with my enjoyment of my vehicle.
It's not like I'm driving a 10 year old beater.
Very disappointed, I will take it to another dealer when I have time.

Also sounded like the engine was binding up when I cranked it over. Had it towed to the dealer, they haven't found a problem yet.
Maybe it is the same noise? I didn't even drive the truck all weekend, truck has 9600 miles bone stock.

Mine wasn't so severe that i wouldn't drive it, didn't make any noise just cranking it
Was your truck tuned.
I hope you have better luck than I did.
