Default Where can I find......

an adapter to place between the oil pan and the oil cooler line block with 1/8" threaded port? There are plenty to replace the oil cooler line bypass cap but looking to keep the oil cooler set up the way it is.

Are you just looking for something you can get an oil feed from? If so you could use something like this: http://prosportgauges.com/oil-filter-adaptor-plate.aspx

I have one of those but would rather not go that route. I really want something mounted between the pan and cooler lines.

Your cooler lines connect to your oil pan?

Is this on the H2 or H3?

I would have thought it would be the same as the standard 6.0 lines that connect to the thermostat block, which bolts to the side of the block above the oil filter.

Lingenfelter has something like this. Do a google search for lingenfelter oil cooler adapter. Don't know if it works with the stock lines though.

It is for the H2. They do mount to the oil pan above the filter. See pic.

I doubt you'll find anything for that. I do like the solution that aaronmc posted, even though you don't want to go that way.

I really should pay more attention when I'm under the truck. I still think in GenI SBC form factor.
