k2500 backfiring thru exhaust, please help!!

k2500 backfiring thru exhaust, please help!!
1999 K2500 6.0 , under a load it will start to break up and pop thru the pipes, let off of gas and get back into it, it doesn't do it until it shifts. it also surges thruout rpm range. any ideas????????
