Default Torque Converter Failure?

Hey guys, just recently finished my lq9 swap. Still running the stock 4l60e but with a 3600 rpm precision vigilante (3rd and 4th were gone in the first 10 miles) first and second seem to still be strong from driving in the neighborhood and up the street. well this morning i drove up the street to the corner store (maybe a mile) and at about 5k rpm it sounded like my y pipe was slipping off the header, i look back and see smoke so i whip back to the house and look up under the truck and see trans fluid coming out of the bellhousing. Im assuming it was my torque converter failing? trans had plenty of fluid before leaving the house? anyway plans are to restall to 3200, stall speed did not need to be 3600 like my shop recommended and a built 4l60 because the truck is a 99 and the i dont wanna f** with the harness anymore.

I'm thinking a line broke as well. Or maybe the converter wasn't seated all the way on the input shaft. It would take a lot more power than that to crack that converter if installed properly I would hope.

There is an oring around the input shaft right? Doesn't the converter seal against that?
