Default 2000 f350 7.3L heavy duty work truck

My shop was about to put a new trans in ^^^^^ truck, and we saw that it had a broken bellhousing and that 2 of the driveshaft bolts had been vibrated out at the front of the driveshaft.

It is 4wd.

Anyone know what might have caused the bellhousing to break or the driveshaft to vibrate bolts out? This is the 2nd or third time it has done this, the 1st 2 transmissions were replaced by FORD and now it has done it again but the guy lost his receipt so he brought it to us.

Any help would be Greatly appreciated.
