I had similar surges and cold stalling. Idle tuning took care of most of it. Still have a bit of an extreme cold stall.

Do you jump in and turn the key from off straight to start? Don't know if it would help but, I always turn the key on then put on my seat belt, grab my sunglasses etc....gives the fuel system a few seconds to prime, then start the truck

not that it is relevent, those mods in your sig, did maggie take those into account? 4.56 gears? just asking.

it's more than likely not gonna be tune related, unless your are hiding the fact you have a cam or something. If you are close to Brian@JPA swing by and throw a rock thru his window after hours and go inside and steal any left over radix parts and mail them to me (especially a rail crossover) then come back the next morning and ask him to help you out.
