Have you played with B4603?

You can set B4522 to say 800rpm to keep it from dying at low speeds (band-aid fix). It won't help the surges though.

I suggest e-mailing your tune to BlackBear (see sponsors--->)
He uses EFI Live and might be able to find the problem, for a fee of course, if it is in fact in the tune and not a giant vaccum leak somewhere.

Try adding idle air to the base running airflow a little at a time. By the way, it took me until now to figure out that the word in your title is "retarded" misspelled. Don't do that anymore.

When you ask if I have played with B4603, are you suggesting I raise the idle speed? The idle was initially set at 950 hot idle, and it was still doing the surging thing.

Oops. I'm "retraded"...

The funny thing about the idle air flow base table, is I can adjust it, but then the idle trims keep pulling air out and dropping it down.

How about posting up a tune and or log file so we can maybe get a better idea of what's going on.

Did it just start or after you installed the mods...I'm not boosted but my truck had a bad idle problem..sometimes shut off while hitting the brakes and turning at the same time...I cleaned the tb and mafs and it ran like a new truck...maybe this will help

I'll try to get a log and tune posted in the next few days.

The truck has a little history. It was totaled and rebuilt. While it was at the body shop, it had some exhaust work done. Here's the progression.

Drove 2 miles from body shop to friend's house to install long tube headers.

Ok after this.

Drove 5 miles from friends house to muffler shop for dual exhaust.

Ok after this.

Drove 3 miles from muffler shop back to body shop.

Ok after this.

Took a couple of weeks to finish truck. After it was done, my dad drove it 4 miles to his house.

Still ok after this.

A few days later, I got in it to drive it back up to the body shop, and that is when it was doing the weird idle. The only thing I can think of that might have been different from the previous times it was driven is the weather was a little warmer. Other than that, I can't think of anything that changed.
