Default truck knocks on start up

So a guy stopped by to pick up some parts, and asked if I could figure out why his truck sounds funny on start up . . .

He cranked it up, and it sounded like an old school motor with a starter gear or bad flex plate gear . . . .. really sounded like crap.

It's a 03 with a LQ4 swap . . . with a small cam . . .
He said that he has a custom tune by a local tuner

I hear of a lot of diesel "Tuners" thinking that gas motors run the same way. So they just tune the injectors wide *** open thinking thats all you need to do. Did his rings wash out? I would say it would make a nice flame thrower with a spark plug welded in the exhaust but with that much fuel the exhaust would likely just blow up!
