what are the specs on the amp?

for example sony amps say they are a 1200 watt amp, however the maybe push a true 400 or so watts. just because your amp says its big doesnt mean that its actually pushing the watts it says.

there are a number of things that could be wrong here... if they bridged the amp too you may be putting too much power to the subs.

JL, Kicker, Alpine are good... there are alot of brands that alot of people dont know about too because they arent advertised also. Like Treo f/e. do some research on stuff before you purchase it just my .02
Treo is Bad ass. I had An Treo SSI 10 powered with a Zapco 750.1 reference series. And it was a pretty bad ass set up.

I'm not sure what all they have it set to, they did the instal and everything. Its a 12 in a sealed box. The amp puts out 570 RMS, sub only take 350 RMS so gains SHOULD BE way down. It's free replacement under a 2 year warranty, so i'm ok for now.

I would say less amp or more speaker. Your putting too much power to that sub even with your gain turned down.

sometimes less amp can cause more issues...say you have less power, people then crank the gains, or bass boost to get 'more' when they are actually clipping the signal.
I see no issues powering a speaker with way more rated power, as long as it's a clean signal.
Being sealed, woofers tend to be more forgiving. (able to accept more power) so I say back gain down a tad, and look at bass boost.

I had a pair of excelon subs a couple of years ago they blew pretty quick. the excelon stuff that is good is the head units all their other stuff is ok but not good. SoundStream, Critical mass , D/D, treo , T3 audio and Atomic. all make good stuff. If I was going to go mainstream audio it would be JL.
